Your completed registration form and payment must be received together by June 20. After June 20 please register at the conference. Registration forms received without payment will not be processed. Cancellations must be received in writing by June 6 and are subject to a $100 processing fee. After June 6 registration fees are non-refundable. You may substitute another person at anytime by submitting written authorization.
